Practice Policy
In line with modern practice, we place an emphasis on preventative medicine, high quality management of long term conditions, good access, encouragement of self care and on delivering a service sensitive to your personal choices whenever possible.

Patient Registrations
If you wish to register with The Orchard Practice please download the following two registration forms, fill them out and return both of them to the surgery. Alternatively, pop in to the surgery and fill both forms out. Don’t worry if you do not have all the information available, you can take them away and drop them back in at a later date. Once you have registered you can see any doctor at the practice, you will also be offered a new patient check up with a health care assistant should you wish to have one.

How we use your data

We provide a confidential service to everyone, including under 16 year olds. This means that you can tell others about your visit, but we won’t. The only reason we might have to consider passing on your confidential information would be to protect you or someone else from serious harm. We would always try to discuss this with you first. If you do have any worries about confidentiality, please feel free to ask a member of staff.
